Pediatric Associates of the Northwest wants to hear from you! Your feedback is important to us as we continue to ensure we are caring for our patients as best as possible. Consider taking our survey! It takes about 10 minutes to complete. 

Pediatric Associates of the Northwest is a proud member of the Children’s Health Alliance and Children’s Health Foundation, an association of pediatricians that work together to improve children’s health all across Oregon and SW Washington. We are collaborating with the Children’s Health Alliance to conduct this survey. 

Each family that completes the survey will be entered to win one of five prizes. Surveys are open to all parents/guardians of PANW patients and must be completed by June 3rd, 2022. 

 

Your privacy is protected. All information that may identify you or your family will be kept private. We will not share your personal information with anyone without your consent. Responses to the survey are also completely confidential. 

 

Participation is voluntary. Patients may choose to complete the survey or not. If you choose not to, your health care at our clinic will not be affected. 

Prize Drawing Rules:

Each family may only be entered into the drawing once. Children’s Health Alliance and PANW employees are not eligible to win a prize. Winners will be chosen at random from all valid entries. Winners will be notified via email and/or phone after the drawing takes place on June 15th, 2022.
