For parents who have children with chronic health concerns or serious injuries, the road to wellness can sometimes involve a few detours. That’s why Pediatric Associates of the Northwest offers complex care management services. From developing a comprehensive care plan, to coordinating appointments among specialists, to simply checking in with families to see how they’re doing, our trained and compassionate nurse care managers are committed to improving the health outcomes of patients.

No two patients are the same and neither is the care offered by PANW. Whether your child has a chronic and complex healthcare need or has recently experienced an acute concern like a traumatic injury, our nurse care managers can help.
Our team collaborates with many children and parents who have medical complexities, such as:

- Cerebral Palsy
- Depression/anxiety
- Developmental delays
- Diabetes
- Down syndrome
- Eating disorders
- Seizures
We also work closely with foster families and children who need an individualized education program (IEP) or 504 Plan.

How We Can Assist You
Pediatric Associates of the Northwest is committed to alleviating the stress and isolation that patients with complex care needs can sometimes experience. In addition to offering individualized support and care plans, our nurse care managers:
- Guide patients and families to plan for now and the future
- Identify community resources
- Secure special equipment as necessary
- Support foster families
- Coordinate care with multiple providers, home health agencies, and community services such as school and county disability resources
- Collaborate with medical team providers and outside resources
- Advocate for patients and families
- Check in frequently with families to assess needs and resources

That’s why our collaborative pediatric team works to identify patient-driven and patient-specific goals—and then develops a roadmap to get there. Our nurse care managers work as integrated team members at PANW while also being fluent in coordinating care with outside specialists and identifying resources for families. We are here to walk beside you, advocate for you, and empower you so that you can worry less and “family” more.
